The applicant shall dedicate the proposed Emerald Hill <br /> Lane to the Town of Los Altos Hills as a Public Street. <br /> 2. The applicant shall dedicate to the Town of Los Altos <br /> Hills that portion of Prospect Avenue which fronts the <br /> site to a half street width of 30 ' . <br /> 3 . The applicant shall dedicate to the Town of Los Altos <br /> Hills that portion of Stonebrook Drive which fronts the <br /> site to a half street width of 30 ' . <br /> 4. The applicant shall record an easement for Pacific Bell <br /> telephone lines and maintenance to the satisfaction of <br /> Pacific Bell . The Final Map shall reflect this easement. <br /> 5. A Conservation Easement shall be recorded for that area <br /> shown on the Tentative Map. The Final Map shall reflect <br /> this easement . <br /> 6. The applicant shall dedicate to the Town of Los Altos <br /> Hills a 10-foot wide pathway easement along Stonebrook <br /> Drive as shown on the Tentative Map, to align with to <br /> pathway easement on the Lands of Kava . The Final Map <br /> shall reflect this easement. <br /> PUBLIC UTILITIES: <br /> including cable tv <br /> 7. All utilities/ are to be under rounded , per Municipal Code <br /> 9-1 .1105.
