HomeMy WebLinkAboutEl Camino Hospital 1 (CI Et Camino Hospital' or OIL HOSPITAL Or SILICON VAt It Service Agreement Background Active Zones is an El Camino Hospital wellness Initiative designed to help organizations create a robust culture of wellness that supports the well-being,morale and productivity of employees.El Camino Hospital(EC H)will provide all of the resources necessary to(1)conduct a comprehensive wellness audit,(2)facilitate the adoption of wellness guidelines,and (3)provide effective tools that encourage and measure outcomes.It is ECH's intent to gather and share Active Zones data with all program partners so that we can learn from one another as we collectively strive to make our employees-and our communities-healthier. This Is a Memorandum of Understanding made this date, 12/12/2013 by and between El Camino Hospital and Town of Los Altos Hills hereafter called"Client," $1 Camino Hospital agrees to the following; • Train the Client and established Wellness Committee on guideline adoption,best practices, communication,reporting procedures,and periodic guideline enhancement ideas,and recommend steps to successful wellness culture adoption. • Conduct a Wellness Culture Audit to help facilitate the Wellness Culture process. • Provide an online Health Risk Assessment(HRA)tool to determine current status of employees'health as well as changes in health over time. Employees/participants will be informed that aggregate(not personal)data will be shared with Active Zones collaborative. • Provide consultation to draft and implement Wellness Guidelines developed by the Wellness Committee. • Ensure that the Wellness Guidelines include but are not limited to the following components: healthy food,beverage,and physical activity guidelines. • Provide a wellness web-portal for one-year.Organization is responsible to pay for wellness web-portal (if desired)after year one. • Provide consultation to implement a wellness platform for data collection and employee engagement. • Provide quarterly Active Zones Collaborative meetings and share best practices. • Provide access to additional fee-based services such as biometric screenings(if desired). • Guarantee preferred vendors are fully vetted and screened. `r • Client agrees to the following. • Establish a Wellness Committee by January 31,2014 to develop and implement Wellness Guidelines. Wellness Committee agrees to meet monthly for first 6 months. • Draft Wellness Guidelines by March,2014,that include but are not limited to healthy food, beverage and physical activity guidelines. • • Conduct a pre/post organization-wide HRA by February 2014/I>ebruary 2015, • Provide EI Camino Hospital with a copy of the final Wellness Guidelines by May, 2014. • Agree to send a company representative(s)to quarterly Active Zones Collaborative meetings starting with the kick-off meeting on February 7,thereafter May,August and November,2014. • Share aggregate data with Active Zones Collaborative for research purposes.
