Dear Mr. Cahill,
Pursuant to the Lease Agreement, J5 Infrastructure Partners, on behalf of AT&T Mobility, hereby
requests Landlord's consent to AT&T's proposed minor modifications and technology upgrades
to its facility located at 27400 Purissima Road, Los Altos Hills, CA 94022. The scope of work
will entail the following:
• Remove and replace two (2) existing panel antennas at each sector (Total -4)
• Relocate one (1) existing panel antenna at each sector (Total -2)
• Remove one (1) existing remote radio unit at each sector (Total -2)
• Install one (1) proposed remote radio unit at each sector (Total -2)
• Install one (1) proposed surge suppression unit at antennas
• Remove eight (8) lines of coax running between equipment area and antennas
• Install one (1) power trunk and one (1) proposed fiber trunk from equipment area
to antennas
• Install one (1) proposed diplexer at each sector (Total -2)
• Various baseband upgrades at equipment area
All work will be done in conformance with the Agreement and in coordination with the Landlord's
requirements for work performed at the property. Plans and equipment details for the proposed
modifications are enclosed for your reference. If you have any questions, please contact me by

If you consent to the proposed modifications, please sign the acknowledgement on Page 3 and
return it to me at the above referenced email address. Please note that AT&T's civil vendor will
contact the landlord ahead of construction to discuss the proposed work schedule.

My name is Chase Otis and I am the J5 Infrastructure Partners project manager leading this project on behalf of
AT&T. My colleague, Tami Drew, advised that there has been a bit of confusion regarding our request for consent. I am
writing today in an attempt to resolve any open questions you may have. First, please be aware that our current request
for consent stems from the requirements found under the operative Lease Agreement for this site. Indeed, AT&T is
contractually required to obtain LL's written consent for the proposed modifications. As mentioned by Tami, we have
had to obtain consent for similar modifications in the past, and you have been the party that has executed such
consents. We have attached one of these past consents to this email as an example.
AT&T's consent requirements are separate from the zoning and building review/approval process AT&T is in the process
of completing. Indeed, the leasing consent must come directly from landowner, here the City Manager's office, whereas
the zoning and building entitlements must come from the respective Departments per the operative local
Code. Therefore, even though we have submitted to the building and zoning departments for approval, we still must
obtain consent to the modifications from your office directly to comply with the lease requirements.
If you would like to have a call to further discuss this matter please advise. Further, I have cc -ed my AT&T Manager on
this email if you would like to ask her for clarity. Otherwise, we would greatly appreciate your executing and returning
the attached consent document dated
